We are in the process of getting ready for the up-coming Basketball Season for Term __ 2019.  I you are interested in competing for Fairfield College please register your interest by completing and submitting the Registration Form below.

GENERAL INFORMATION:

• All Games will be played at ________________________

• Juniors must wear __________________________

• All students must arrive at the fields 30mins before their scheduled game times.

• Competition runs from ________________________

• Students are to find their own way to and from the fields, transport will not be provided.

• All students are expected to be available for all games, and must be committed to trainings on __________ 3.15pm - 5pm at _________________

By submitting the following Registration Form you:

• Give permission for your child named below, to participate in a Fairfield College Basketball Team.

• You are aware that games are played on ______________________, and that it is your responsibility to ensure that they get dropped off and picked up from the fields safely.

• By submitting this form, you are confirming that your child will be committed to trainings and games, and that if your child is not committed they may be asked to leave the team and will still be liable to pay fees due.

• Students are expected to follow the school codes of conduct at all times.
